The Blended Case Manager provides care coordination across multiple life domains on behalf of people receiving mental health services in support of their process of recovery.

This is an excellent opportunity for current BCM's and for recent or soon-to-graduate individuals looking for relevant experience in the Mental Health field. Ideal candidates will have a degree in sociology, social welfare, psychology, gerontology, anthropology, other related social sciences.
Job Responsibilities
• Manage a caseload of approximately 20 consumers
• Maintain positive working relationships with individuals, supporting and advocating for their identified goals and needs
• Coordinate care across all life domains
• Work with professionals from multiple agencies and systems
• Develop, implement and review service plans in collaboration with service recipients and family members/friends utilizing shared decision-making tools and techniques
• Accurately complete and submit paperwork in a timely manner according to Federal, State and MCO regulations and internal billing requirements
• Meet productivity expectations with most of the work performed in the community as billable activity
• Participate in regular program, team and individual supervisory meetings and mandatory agency training
• Participate in intermittent daytime, evening and weekend on-call responsibilities
• Assist consumers as needed in carrying groceries, moving, pushing and/or lifting wheelchairs or other physical activities
• Adherent to all laws related to Medicaid Fraud, Waste and Abuse and compliance requirements
• Participate in and advocate at court hearings i.e. County or Federal Court, District Justice, Social Security, Child Welfare, Housing

This position is designated as a "targeted role" within the Erie County Human Services Cohorts (HSC) initiative. Candidates selected for this position may qualify for HSC participation and related financial incentives, subject to program and eligibility requirements. Participation in the HSC and approval of financial incentives are determined by HSC program administrators and are not guaranteed by the employer.

Experience Requirements

Qualifications
• Education:

1. Bachelor's degree with major course work in sociology, social welfare, psychology, gerontology, anthropology, other related social sciences, criminal justice, theology, nursing, counseling, or education; or,
2. Registered nurse; or,
3. A high school diploma and 12 semester credit hours in sociology, social welfare, psychology, gerontology, or other social science and two years' experience in direct contact with mental health consumers; or,
4. A high school diploma and five years of direct mental health care experience in public or private human services with employment as a case management staff person prior to April 1, 1989.

• Work Experience:

Mental health direct care experience is working directly with adult or children mental health service consumers, providing services involving casework or case management, individual or group therapy, crisis intervention, early intervention, vocational training, residential care or social rehabilitation in a mental health facility or in a facility or program that is publicly funded to provide services to mental health consumers, or in a nursing home, a juvenile justice agency, or a children and adolescent service agency.
