Assertive Community Treatment (ACT) program: Dynamic & rewarding full-time position working in community mental health with team of colleagues including the psychiatrist serving a shared caseload in community; ACT Team Nurses assist with coordinating medications between program psychiatrist and client's PCP's and medical specialists with pharmacies, clients and/or team members assisting clients with learning medication management skills. Nurses also assist in coordinating & communicating between behavioral health and medical providers to assist the client with pursuing the best health outcomes. Program RN's have assigned clients to meet with each month for routine nursing visit and/or to administer long-acting injectable psychotropic medications; will assist with linking & supporting clients as appropriate to community medical appointments; also is able to be flexible for unexpected needs which can arise within the team & shared caseload. Daily organizational meeting with full team allows all staff to be aligned on treatment goals, as well as emerging issues to address proactively. ACT is an Evidence-Based Model which has proven efficacy for individuals whose primary issues are related to diagnoses of Major Mood Disorders and/or Schizophrenia or related psychotic disorders.
Once orientation period is complete (at least 3 months), candidate may elect to adjust M-F 8-4 schedule to allow for more flexibility if it suits program & client needs.
ACT staff cross train teammates on psychiatric diagnoses and medication interventions, nursing and physical health matters, therapy / therapeutic interventions, skills-based /psychiatric rehabilitation interventions, peer-support, vocational and employment services and case management services, permitting ACT staff to learn a great deal about community mental health care and service in Erie County.
Licensed as Registered Nurse in Pennsylvania
* Experience working with individuals with mental health disorders is preferred, but not required.
* Valid PA Driver's License
* Must pass pre-employment physical examination
